Person-Directed Planning · Ages 0–18

Every Path Forward Starts With a Plan

Person-directed planning for individuals with disabilities, ages 0–18. We walk alongside families to build futures rooted in possibility.

Planning With You, Not For You

At Accessibility Consulting, we believe every individual — regardless of ability — deserves a future shaped by their own dreams, strengths, and voice. Led by Nisha M. Khan, a dedicated Person-Directed Planning (PDP) consultant, our practice partners with families, caregivers, and young individuals to navigate the critical transitions of life from early childhood through age 18.

Our approach is deeply personal. We listen first, plan together, and advocate always — ensuring that every step forward reflects the unique potential of the person at the centre of the plan.

Our Mission & Vision

Mission

To empower individuals with disabilities and their families through person-directed planning — providing expert guidance, compassionate advocacy, and practical tools that open doors to a life of dignity, inclusion, and self-determination.

Vision

A world where every individual with a disability is seen, heard, and supported to live a full and meaningful life — where planning is a bridge to possibility, not a barrier to it.

How We Support You

Comprehensive, person-centred support across every stage of life from birth through young adulthood.

Transition Planning (Ages 0–18)

Structured, forward-looking planning that supports individuals and families through every major life transition — from early intervention through school years and into adulthood. We map the journey so no milestone is missed.

Person-Directed Planning (PDP)

A strengths-based, individual-centred planning process that puts the person with a disability at the heart of every decision. We facilitate conversations, build circles of support, and create living plans that evolve with the individual.

IEP Support & Advocacy

Navigating Individual Education Plans can be complex. We support families in understanding, contributing to, and advocating for IEPs that truly reflect their child's needs, goals, and capabilities.

Goal Setting & Life Planning

Helping individuals and families articulate meaningful short- and long-term goals — covering education, employment, housing, relationships, and community belonging — and building actionable plans to achieve them.

Family & Caregiver Guidance

Families are the backbone of every plan. We provide guidance, resources, and a supportive space for caregivers to build confidence, reduce overwhelm, and become powerful advocates for their loved ones.

Community Resource Navigation

The landscape of disability services and community supports can be overwhelming. We help families identify, access, and connect with the right resources — from government programs to local community networks.

What Families Are Saying

Real experiences from families we've had the privilege of walking alongside.

Nisha helped us navigate the IEP process with confidence. For the first time, we felt like true partners in our son's education — not just observers. Her guidance was clear, compassionate, and incredibly practical.

Sarah M.

Parent of a child with autism, age 9

We had no idea where to start when our daughter turned 16 and transition planning became urgent. Nisha sat with us, listened to our fears, and helped us build a real plan. We finally feel like we have a roadmap.

James & Priya T.

Parents of a young adult with Down syndrome, age 17

The Person-Directed Planning sessions were transformative. Nisha has a gift for drawing out what matters most to our son — in his own words. The plan we built together actually reflects who he is.

Linda K.

Parent of a child with cerebral palsy, age 12

I was overwhelmed trying to figure out which government programs our family qualified for. Nisha cut through the complexity and connected us with supports we didn't even know existed. She's an incredible advocate.

Marcus O.

Father of a child with a developmental disability, age 7
